Hi Hershey Elementary Parents / Guardians,

I hope this email finds you and your family healthy!  This online registration prevents the Nursing Offices from connecting with you about items we need for our summer preparations and the start of the 2023-2024 school year.

Per the Pennsylvania Department of Health’s School Health program, students need a dental exam, physical exam, and vision screening before/during Kindergarten. Immunizations must be completed by the start of Kindergarten.

If your child has a dental appointment, well-child check, vision screening, and immunizations scheduled this summer, please ask the provider to complete the information. Links to the forms are below. Please send/email the completed forms to me.

If your child is going to need medication at school for 2023-2024, you can use one of the attached forms for asthma inhalers and epinephrine auto-injectors.  You also can use the general form for any prescription or over-the-counter medications or ask your provider to write the med order on her/his own prescription pad or notepaper.

At this time, Medication Drop-Off day is scheduled for: TBD - will be the week prior to the start of school in August 2023.
